The Quick Medical Assessment (QMA) includes all the residents' information required in an emergency situation.
The information will include the name, address, age, medicare number, allergies, current medications,
living will (where applicable), and family member(s) to contact in case of emergency. This information will be available to family members online
and in PDF format, for quick and easy printing, to attach to a resident in the event of a medical emergency. This ensures that emergency medical personnel have
a quick and accurate snapshot of the resident's medical status. |
| Wellness Assessment |

The Wellness Assessment will provide family members with a quick overview of the general well- being of their family member.
The assessment would include 5 categories rated 1 through 5, to assess appetite, sleep pattern, state of mind, responsiveness/communication and participation with
an area to add notes, if so desired. |

The online forms section would include all legal documents including leases, lease amendments,
eligibility for government assistance, tax receipts, etc. available in PDF format. |
| Invoice & Payment Records |

These reports would include a record of all monthly invoices and payment records. |

This page will display pictures of family members participating in activities and special occasions
such as birthdays, parties, musicales, or just sitting on the patio enjoying the summer sun. There could also
be a monthly agenda of activities and events. |
